Problem with CPU usage - Disk io rate
Since about a month ago my website CPU usage and Disk io rate has jumped significantly (I run a linode 1024):

Assuming you allocate 40% of your ram to apache this allows ~50mb per php process, you have php's memory limit set to 128mb but I doubt every request uses that so 50 should be enough.
Also you should look up modexpires and enable gzipping of your javascript files (modgzip)
